1.) From the Client LaunchPad, select the “My Resources” tab, then go to the Resource Directory.

2.) Press “Intakes” and answer all of the questions in the “CommuniCare Intake” which can be selected from the drop down, and press Find Services.

Note: The Wyoming CommuniCare Intake is a quick and convenient way to collect information about a client’s needs and risks so they can be connected to services immediately.
