If a client wishes to deauthorize consent for a specific agency, follow these steps:
1.) Start by navigating to the Client Profile of the individual who wants to deauthorize consent.
2.) Next, under the “Client Shared Profile” tab, you will go to the Consent form.

3.) Next you’ll be taken to the signed consent form. Click the three-dot menu next to the document and select “Deauthorize Entity.”

4.) You’ll see a list of all current Care Team members. Select the correct agency and follow the prompts to complete the deauthorization process.

5.) Send a Care Team message to the remaining members to inform them of the client’s request and the change made.
6.) Remember to call the client to let them know the deauthorization request has been completed.
